Caregiver Relief Grant
This Caregiver Relief Grant has been established with funds from the Alzheimer’s Foundation of America to serve the enhanced needs experienced by caregivers of individuals living with Alzheimer’s and other forms of Dementia (cognitive caregivers). During crisis situations, such as the COVID-19 pandemic, where economic recession or depression exists, caregivers are particularly vulnerable and often overlooked in government and organizational efforts to distribute economic support. During the COVID-19 pandemic, when in-home respite and companion care is unavailable and the Dementia population and their caregivers are particularly at-risk, funding support should be made even more accessible. This fund will provide 20 cognitive caregivers and their families with $250.00 to help reduce the financial burden and anxiety that caregivers can experience every day and particularly during this unprecedented time.

Application Instructions
Please fill out the application (below). The first four sections are required, but will not be scored. In order to qualify to receive funds, all sections of the application must be filled out and residence, caregiver, and diagnosis requirements met. The final part of the application can be written as a cohesive essay or short answers, with a total of 500 words. This section is the scored portion of the application, with up to 20 points awarded for each question based on the need expressed and the quality of the story told. This will be up to the discretion of the Grant Review Committee.
